| Description: | The Psychosocial Rehabilitation Program provides outpatient psychiatric services to adults with severe and persistent mental illnesses. The needs of this population include on-going education regarding psychiatric symptom management, enhancement of social supports, assistance in finding alternative housing, linkage to educational resources, assistance with obtaining employment, referrals to resources for physical health, and assistance with obtaining financial entitlements. The program is community based and utilizes the concepts of recovery which include hope, client self-determination, empowerment, focus on wellness, and developing a sense of purpose and value.
Key service modalities include case management, skill training and development, therapeutic behavioral services, individual counseling, family counseling/education, and crisis management as needed. Family and community providers are routinely consulted as part of the on-going evaluation and treatment. |
