| Description: | Non-profit providing affordable primary health care services for low-income, uninsured individuals. Patients eligible for Access to Care must:
Live in SUBURBAN Cook County or northwest Chicago (west of Pulaski Road AND north of North Avenue)Have family income less than 200% of the federal poverty guidelines.Have no health insurance (or a deductible of $500 or more per person) and be ineligible for Medicare, Medicaid, All Kids, or FamilyCare. Registered patients are referred to partcipating doctors.
