Need tax forms, help or just information? Click below for all the info you'll need before April 15th!  Note: Although the Library does not get  IRS or State of Illinois tax forms., you can  download and print them from the web at any Library location for $0.20 per page.


Tax Forms - Online

Nearly all tax forms are available on the Internet.  Federal and state tax forms are available online to download and print.

 

Internal Revenue Service (IRS)

Access federal tax forms, publications, and other related information on the IRS website.  Visit 1040 Central for some of the most common tax forms and questions.  Speak Spanish?  Visita el IRS en EspaƱol.

 

Illinois Department of Revenue (IDOR)

Find Illinois state tax forms, publications, and other related information for individuals, businesses, non-profits, and more.


Out-of-State Tax Forms

The Federation of Tax Administrators provides links to other state revenue departments, which may offer online tax forms for their respective state. 



Tax Forms - Print

Order Print Forms by Phone

Order many of the current year tax forms and publications for delivery by U. S. mail. 

For Federal tax forms, call 1-800-829-3676. Delivery is within 7-15 days of receipt of request.  

For Illinois tax forms, call 1-800-356-6302.  Delivery will take 7-10 days from time of request.

 

Evanston Public Library 
The Library no longer carries tax forms. Forms and publications can be printed from the library computer stations on the 2nd and 3rd floor for 20 cents a page. Ask for help at the 3rd floor Reference desk.

Northwestern University Library 
The Northwestern University Library no longer carries tax forms; however, Government Depository staff will help patrons print forms from the Internet. The Depository is located on the 1st floor of the library and is open to the general public.

Skokie Public Library 
A large number of state and federal print forms and publications are available on the 2nd floor.  Forms and publications may also be printed from the Internet for 10 cents a page.

The Taxpayer Advocate Service is an independent organization within the IRS.  If you have tried to resolve a tax problem with the IRS and are still experiencing delays or are facing economic harm, you may request the assistance of the Taxpayer Advocate. Tax Assistance Program-Ladder Up
Free assistance for low income taxpayers, at locations throughout the Chicago area. All sites operate on a first come, first served basis.

Most sites are open Saturdays, January 22 through April 16, 8:30 - 11:30 a.m.; all sites closed on March 5.
Some sites also offer services on Tuesdays and Thursdays, 5:30-8:30 p.m.

CEP provides low-wage workers with free, high quality tax preparation and an alternative to paid tax preparers. Through their tax clinic, they also provide free, professional representation to low-wage taxpayers facing an IRS controversy; and they offer tax training for small businesses.

The Chicago Kent College of Law Tax Clinic is a full-service federal tax controversy practice, representing taxpayers who cannot afford to hire a private attorney.  The Tax Clinic does not provide general assistance filling out tax forms.

Levy Senior Center 
Seniors can schedule tax assistance appointments at the Levy Center by calling 847-448-8250. One-hour tax help appointments for simple returns will be provided on Tuesdays and Fridays.
