Description
Comprehensive, compassionate and multidisciplinary care program that focuses on individuals coping with memory problems. The team consists of a neurologist, geriatrician, neuropsychologist, registered nurse and licensed clinical social worker, all specializing in memory disorders. The goal of the program is to diagnose, treat, and manage the patient's care in conjunction with his/her primary care physician.
Family Support Group meets the second Thursday of every month at Glenbrook Hospital.
