Heartland Health Centers is a federally qualified health center organization serving Chicago’s north side. Last year, our ten locations provided nearly 21,000 patients with a medical home where they could receive comprehensive primary care, oral health care, and mental health care services. We’re open to the entire community – the uninsured as well as those who have Medicaid, Medicare, or private insurance. We offer free interpretation services in over 35 languages and ensure that care is provided in a culturally competent way. Our sites include four community health centers, four school based health centers, and two sites co-located with agencies providing mental health and substance abuse services.
